All patients referred from a GP or hospital will be assessed for NHS funding (once the registration questionnaire has been returned).
Not all patients are eligible for funded treatment, whilst specific criteria vary from one Primary Care Trust (PCT) to another. Below is a general summary of the eligibility criteria.
